Our Care Process at 24/7 Expert Care

Our care process is designed to ensure adult domiciliary care is delivered safely, consistently, and in line with regulatory standards. Every care package follows a structured pathway, from initial referral to ongoing review, so individuals, families, and professionals know exactly what to expect.

We provide care exclusively for adults aged 18 and over across London and Berkshire.

Care begins with an enquiry or referral. This may come from an individual, family member, local authority, or health professional.

At this stage we:

  • Gather key information about care needs
  • Confirm eligibility and service location
  • Identify urgency and initial risks

This allows us to respond promptly and appropriately.

All enquiries and referrals are reviewed to ensure we can safely meet the individual’s needs.

We confirm:

  • Adult eligibility (18+)
  • Care is required in the individual’s own home
  • Location is within London or Berkshire
  • Initial risk and safeguarding considerations

If care cannot be safely provided, we are clear and transparent at this stage.

A structured needs assessment is completed before care begins. This assessment focuses on safety, wellbeing, and individual preferences.

The assessment includes:

  • Daily living and personal care needs
  • Mobility, medication and support requirements
  • Risk and safeguarding considerations
  • Individual routines, preferences and goals

     

Assessments form the foundation of safe and person-centred care.

Following assessment, a personalised care plan is developed. Care plans are clear, practical, and outcome-focused.

Care plans include:

  • Agreed support tasks and schedules
  • Identified risks and control measures
  • Individual preferences and routines
  • Review dates and outcome goals

Care plans are shared with relevant stakeholders where appropriate.

We carefully match trained, DBS-checked carers to each care package based on needs, skills, and compatibility.

At the start of care:

  • Carers are briefed on the care plan
  • Care delivery is closely monitored
  • Additional oversight is applied during the initial period

This ensures care starts safely and smoothly.

Care delivery is monitored continuously through supervision and electronic care records.

This includes:

  • Real-time visit monitoring
  • Accurate documentation of care delivered
  • Ongoing communication with families and professionals
  • Prompt response to any concerns or changes

Monitoring supports accountability and quality assurance.

Care needs can change over time. We carry out regular reviews to ensure care remains appropriate and effective.

Reviews consider:

  • Changes in needs or circumstances
  • Feedback from individuals and families
  • Professional input where required
  • Safeguarding and risk updates

Care plans are updated as needed to reflect current needs.
