24/7 Expert Care works in partnership with local authorities, NHS teams, and professional referrers to deliver safe, auditable adult domiciliary care.

We accept referrals for adults aged 18 and over requiring home-based support across London and Berkshire. All referrals are managed through a structured, compliant process designed to support timely placements and transparent reporting.

Private referrals are also accepted.

We accept referrals for adult domiciliary care including:

Our Referral & Placement Process

Our referral process is designed to be clear, responsive, and auditable.

  1. Referral Submission
    Referrals are received via our secure referral form, email, or direct contact with the Referrals Team.
  2. Initial Review
    Referral details are reviewed promptly to confirm eligibility, capacity, and risk factors.
  3. Assessment
    A structured needs assessment is completed, including safeguarding and risk considerations.
  4. Care Planning
    A personalised care plan is developed with clear outcomes, schedules, and review points.
  5. Care Commencement
    Trained staff are matched and care begins with enhanced monitoring during the initial period.
  6. Review & Reporting
    Ongoing reviews and outcome reporting are shared with commissioning teams as required.

All referrals are managed within a strong governance framework aligned with the CQC Fundamental Standards.

  • Safeguarding-first policies and mandatory staff training
  • DBS-checked and trained care staff
